Youth Mental Health Program

The Primary Care Network Youth Mental Health (PCN YMH) program has been running in full force for over a year now. We serve youth ages 5-18 and offer screening, diagnostics and treatment through our PCN team. Our YMH nurses complete intake and screen patients for a variety of mental health concerns (ADHD, depression, anxiety etc). They then refer patients to our CAN Reach trained physicians for diagnosis and medication and/or to our Behavioral Health Consultant for short-term therapeutic services. We also offer group courses for parents and youth such as “Chaos to Calm” parenting course as well as a hybrid “Anxiety to Calm/Happiness Basics” course that can be run in the schools.
