Primary Care Network Diabetic Program

My name is Vanessa Stewart, and I am the Diabetic Nurse at the Rocky Primary Care Network. We want to make managing diabetes as simple and supportive as possible. We are excited to offer this service to our physician team and patients. The Diabetic Nurse offers monitoring, support and education on diabetic care. Some specific services offered are:

  • Review diabetic lab results
  • Educate on how healthy lifestyle choices impact on HA1C [1]and blood glucose levels
  • Assist patients in understanding HA1C and blood glucose target levels and what the numbers mean
  • Discuss and assist patients with the use of blood glucose monitoring devices
  • Instruct how to properly administer insulin [2]and non-insulin injections e.g. Ozempic & Manejo
  • Provide ongoing support with insulin and non-insulin diabetes management
  • Forward referrals to other allied health care professionals to further assist with diabetes management (such as dietician and lower limb assessment)

 

This service is FREE, and patients can be referred by a health care professional or they can also self-refer by calling or faxing the Rocky PCN.

[1] A hemoglobin A1C (HbA1C) test is a blood test that shows what your average blood sugar (glucose) level was over the past two to three months.

[2] Insulin is a peptide hormone produced by beta cells of the pancreatic islets encoded in humans by the insulin (INS) gene. It regulates the metabolism of carbohydrates, fats, and protein by promoting the absorption of glucose from the blood into cells of the liver, fat, and skeletal muscles

The program is free and it begins with meeting a diabetic educator to develop a plan that may include:

  • One-on-one diabetic counselling with a certified pharmacist
  • Administering insulin, titrating insulin doses, checking blood sugar
  • Discuss medication management and appropriate therapies
  • Small group sessions and workshops
  • Intro to exercise with diabetes classes led by a degreed exercise specialist
  • Annual foot examination with registered nurses
  • Provide you with diabetic tools such as BG meters, log books and other diabetic supplies
