Chronic Disease Self-Management Program

The term “Chronic Disease” means you have a condition you need to manage and work into your daily routine for the rest of your life. Some types of chronic diseases are diabetes, asthma, high blood pressure or cholesterol problems.

Our goal is to help you manage your health conditions. It may be a specific treatment or general management of a chronic illness. We will work with you to help you understand what your chronic health condition is, and to teach and develop goals with you to help YOU manage YOUR health.

We will be partnering with Alberta Health Services and other groups in our community to round out the team. The most important person of the team is you; to be effective in change in your health we need your input. The next important person is someone(s) you can identify as you support.

Being part of this program means that you will have extra help until you are ready to manage on your own or “graduate” from the program.

Patients can self refer to the Chronic Disease Self-Management Program or be referred by their physician to the PCN receptionist.

Once you have been referred to the PCN the receptionist will offer you a date for your appointment. The receptionist will also register you for group education, through AHS, which is available in Eckville, Caroline and Rocky Mountain House, and online through Zoom.

Appointments are approximately 60 minutes in length and are for whole scale planning of a patient’s needs to self-manage their chronic disease.
