Community Health Navigator

Community Health Navigators (CHN’s) help patients manage chronic health conditions through supporting them in three areas: system navigation, patient self-management, and social connection. CHN’s are knowledgeable in health, social, and cultural resources available in the community. They are trusted members of the primary care team and accompany patients through their health journey virtually, in person, or as a combination.

  • Guide you in planning your health priorities

Referral Criteria for Patients

  • Living with chronic diseases that are uncontrolled
  • Experiencing social or material deprivation with emphasis on social determinants of health
  • Motivate to achieve goals for wellness
  • Unstable mental health
  • Facing barriers and in need of multiple supports
  • Lack of informal support
  • Cultural or language barrier
  • Facing stigma and social exclusion

Exclusion Criteria for Patients

  • Residing in LTC (long-term care), Designated Supportive Living
  • Does not have the capacity, unless accompanied by alternate decision-maker
  • 17 or younger WITHOUT parental consent
