Anticoagulation Clinic

This program provides ongoing monitoring and regulation of patients on oral anti-coagulation therapy. This program targets patients on long term anti-coagulation therapy and they need to be “within range” for 4 weeks minimum prior to being part of the clinic. The program is managed by a pharmacist and this pharmacist will review and monitor the patient’s lab work, make dose changes and teach patients to eventually manage their own care.

Patients accepted into this program must be attached to a physician with the Rocky Medical Clinic and must be referred by that physician.
